Transition to Adult Healthcare (for parents)

Please join Lydia Dennehy of The Family to Family Health Information Center for a workshop to learn how to support your child’s move to adult healthcare. Are you and your young person ready for the changes in health care that happen when they turn 18? For example, do you have new primary care providers or specialists lined up? What about a plan for communication if your young person needs support making medical decisions? Don’t wait until your young adult is almost 18 to prepare. This interactive workshop will give you practical tools for managing health care transitions as well as for preparing your young person to be their own health care advocate. REGISTER HERE
